Do I want to keep my bread in a perforated bag or in a regular plastic bag? Should I put it in the fridge if I want it to last more than a couple of days?

perf is fine, the fridge keeps it fresch a couple extra days only, the freezer will hold it for weeks but I would suggest putting it in a sealed freezer bag, or it'll taste like the freezer which isn't whonderful

A paper bag's better. It should cause it to slowly dry out instead of developing mold. The fridge will also inhibit mold, but will cause the bread to go stale in a day and make it useless for anything but toast.
