It says don't add water or fish stones. Do I just put nothing in it and throw it on the stove? How long would mackerel on med heat take?

Most people in Japan don’t have an oven. A good number have a broiling/roasting “drawer” designed for fish.
Your pan is designed for someone who has neither, wants roasted fish, and is limited to a gas burner.

I think it's a pan and lid that's suppose to mimic a broiler drawer. It's never work because the fish is going to steam though. Also who knew the bottom drawer where you keep all your pots and pans was the broiler drawer!

Yes. That’s why you use it as explained in >>17386248

They usually need to be separated from the drippings. One of the more common fish for grilling/roasting whole in Japan is sanma (aka pacific saury). It’s also the fish pictured on your box.
It’s a rather fatty/oily fish. So it needs to cook on a grate so the skin can get crispy.

Basically yeah.
Standard Japanese preparation would be to oil the fish’s skin and sprinkle with salt, then just roast it whole.
