I used to work there and I could see this happening. Someone might have selected the wrong timer. they must have selected the hamburger patty timer, rather than the quarter pounder setting. but you would have to be pretty clueless to not notice, that they were "ready" way too fast and they would look slightly but obviously different.

I worked there too, generally we would set up one clamshell for hamburger patties, one for QP patties and never reset them. For me it was always hamburger right QP left. I could put 12 down officially but 16 in a crunch for the hamburger patties and 9 with very limited room to cheat for QP (maybe 10).

The numbers you’re saying make sense because the large ones we had would have fit 12 QP but it was 3/4” too small on the backmost row, which left it raw. IIRC there was a configuration allowing 10 with multiple QPs just being on the edge but inside. Gotta say, though, sixteen burger patties at a time was kino.
