There are good molds (like on cheese and aged steak). They are good because the people who sell them know what they are doing and know what cultures to cultivate.

Mold that grows on food that's been out for a long time is potentially hazardous because you don't know what strains of molds you have on there.

In western countries with good food regulations the biggest concern isn’t acute toxicity but chronic exposure to very toxic mycotoxins that can result in increased cancers, kidney damage, birth complications and developmental issues within exposed populations. Aflatoxin B1, ochratoxin A and the various trichothecenes are such examples.

In this sense you are wrong when you state that mycotoxins require a substantial dose to cause harm. There is a reason our governments spend a lot of money regulating and monitoring for mycotoxins in our food supply and it is an important trade issue.

You should avoid mycotoxin exposure whenever feasible which, in our case, ultimately means not eating spoiled or contaminated foods. There is a wealth of literature supporting this basic precaution and knowingly consuming moldy foods is simply ridiculous. The idea that Penicillium = penicillin and therefore moldy bread is fine to consume is also absurd because 1. You do not know what Penicillium species is/are colonizing your bread, 2. Penicillin-producing species (P. chrysogenum and P. rubens) are unlikely to be on your bread and, 3. Even if they are on your bread they are also producing a suite of other secondary metabolites that may be unhealthy. Not only that but there are intraspecific variations in secondary metabolite profiles of fungi as well as the affect of substrate. For example I will gladly eat blue cheese made with Penicillium roqueforti but I would not feed livestock silage contaminated with this species because I know that it produces PR-toxin and other nasty toxins when grown on cellulose. On salty, milk substrates it is a delicacy and on silage it can be deadly.

In conclusion, do not eat mold-contaminated foods.

Certain people might have severe reactions to the bacteria that accompanies certain mold strains, most people will get nausea and/or diarrhea.
If you get really unlucky and eat too much of a bad mold strain you might ingest certain mycotoxins which can in high concentrations kill you.
The chance of that is pretty low though to be honest.
