Sysco and/or some other big name food distribution company like that.
I get that restaurants need to get their food, especially in bulk but some of these services seem to distribute already made foods that the restaurant just needs to thaw and reheat. I've checked out places that are lower-end and use Sysco for instance and I've noticed that certain menu items just taste the same across various establishments.
I guess it's more of a red-flag for common items though, since I have gone to places that use Sysco or equivalent and they still had some unique/specials that felt a lot more scratch made

Be me, walk in the door, and if I smell skunky beer soaking some bar mat and stinking up the place, I just walk out.
Many times upon arrival from my car, I might go prefer to upfront use the restroom and wash up my hands, before sitting down comfy in a booth, but if the bathroom smells or any stall is out of order and not labelled as such, then ugh, I might just assume they don't clean at all or ever walk in there to make sure it's safe. I might even assume lack of paper towels means every employee and customer is touching that exit door handle bare-handed, and then the back of their chair or other places all over the restaurant. Nasty.
Flies inside the restaurant, even just one. Something is wrong. They are propping unscreened open doors at the kitchen, flies are coming off rotting food in dumpsters or animal droppings nearby, or they are leaving garbage in bags to the point that entire maggot-fly cycles can be completed, aka too long.
If the floors are slippery, actually oily, in a restaurant with a fried food theme, like chicken, then i know the entire place was mopped without a single rinse or by zone. A person went quickly once through the kitchen then a cursory trip through the restaurant or even used the same dirty grease logged mop as yesterday. This means there is zero employee safety training or care. All of these care issues translate to kitchen safety, cross-contamination between diners with TB, hepatitus, norovirus, or whatever fecal matter they carried out of the bathroom all day.

Food tourist here , offering some online advice since I usually find hipster restaurants through Google Maps and therefore don't ever have to deal the shithole problems listed above. Assuming you want novel, high-quality food:

These are strong indications that the restaurants positive ratings came from older people, who care more about quality in repetition rather than anything worth your money. Food will be bland or boring.

Putting aside the fact that most fixed price options are a ripoff, any chef offering only one menu is a small timer pretending to be big league. It means they don't have confidence in making anything other than their specials noteworthy. Food will be unusual, but not particularly amazing or well-tested. Lots of wannabees run these kinds of restaurants hoping to evolve into a celebrity chef. 200% chance that the chef will come out and try to get to know you/make friends with the dining party. A good restaurant runs on impersonal, impeccable service and the strength of the cuisine's reputation alone.
