They actually had a 'secret' menu item for years. Any McDonald's I went to I could order a daily double and they always knew what it was despite it not being on the menu for a long time already. I went through a macdalds drive thru (North Carolina) for a coworker something like a year and a half ago and she had me ask for that and they produced it no problem. This was also in a very high volume area to where it's probably one of the most profitable in the area so they might have staff trained much better than average, also evidenced by the fact that if it takes more than 90 seconds they apologize for the wait.

I miss it, if and when I go through there for a quick lunch I'll never get more than a small cheeseburg or McChicken®, maybe a small fry if I'm really hungry, or a big mac really rarely, the mac jr was the best of both worlds. was dissapointed when it vanished, especially since they can obviously still make one but now you have to autistically ask for mac sauce and lettuce and won't get the sesame bun. that's 3x the meat and probably 4 or 5x the price since mac jr uses one small patty, as the big mac uses two small patties. not worth the difference for a sesame bun. I've done it before but it's fucking annoying since they ask three times what modifications you wanted and when they're that unsure it's hard to believe they'll actually get it right. maybe I should use the app but I don't want to. hell I did just that with a mcdouble a couple weeks ago and they gave me a sad dollop of mac sauce instead of actually enough for the small sandwich as if to say fuck you just order le big mac

Most places will do it if it isn't a big hassle or it isn't peak rush hour.
Even if they don't, they probably won't tell you no, but they'll just make the order as normal and just bet you won't make a huge fuss, and even then you can just give them something else for free/apologize/whatever is most efficient. When I worked at MCD we this most often came up with people requesting fresh fries or unsalted fries, with the idea they would be fresh.
Making generic fast food workers put out extra effort for your personal sake is often like trying to get blood out of a stone.

They don't say it, of course, but they routinely fuck it up, and I'm pretty sure it's on purpose. The employees do it of their own volition because it's extra work in a fast paced job that they don't get paid for, and their employers probably also turn a blind eye to the extend that they can (most fast food eating Karen wannabes are too lazy to get out of their car and make them correct the order, much less complain to the manager) because custom orders are actually fucking up their business model, which is built from ground up on speed. Everyone getting their custom orders correctly further encourages them to keep doing it, which slows down the line for the customers behind them who just order off the menu and want their food fast. Everything is stacked against McDonald's custom ordering, which gives me every reason to believe the routine fucked up custom orders are almost by design.

Incorrect. It was 1/2 a pound of beef, or essentially a double quarter pounder with some extra bread, some lettuce and some sauce. They used the same patties as for the quarter ponder. A typical big mac uses the patties of a McDouble which are a little under 1/8th of a pound each (meaning that the meat in a Big Mac = slightly less than the meat in a quarter pounder, or 3.8 ounces to be exact).
