To be fair it's an oversimplification of the truth. Especially for ground beef which is the type of raw sandwich they're talking about. If you want to eat ground or finely chopped beef raw, you need to go get yourself a meat grinder and grind it yourself, then eat shortly after. It doesn't matter whether you're buying from Stop and Shop, Whole Foods, or your local farmer's market. If it's preground you can't really reduce risk substantially. I wouldn't really trust getting the meat ground at a butcher same day either. It'll considerably reduce the risk since all the meat is from one, whole animal, but unless they clean the grinder before they grind yours that's still a higher risk than you'd be exposed to grinding your own.

All it takes is one single piece of meat to be contaminated with parasites or a pathogen and you will be at much higher risk. And your butcher is getting factory farmed meat to begin with. Buy a whole cut of meat directly from a farm whose quality you can be assured of or learn to raise and butcher meat yourself. Buy a grinder attachment for a stand mixer. Then grind the meat the same hour you intend to use it, and make damn sure that the meat grinder is clean before you do. You want safe raw meat? That's how you lower your risk. I guarantee you precious few of the people buying from 'quality' places are doing that.

Especially if you're talking a generic grocery store butcher doing this to get through college and not a specialty one who considers it their craft.

I am aware of what sorts of food safety issues you get from factory farming. Taking meat that may possibly have been contaminated due to unhygienic conditions and serving it raw is not safe. The issue in that case is not that the butcher is at fault, but that he sourced his meat from a location unfit for raw consumption. Let's say one cow is contaminated and then you ask your butcher to grind meat from another cow which isn't on the same equipment, unwashed. Yours is now contaminated too. Whether that's from feces, parasites, or pathogens.

Consider where most people are going to source their ground beef from and why this warning is made. It's an over-generalization, but it's a necessary one. The idea most people have about quality and how to find it is simply not sufficient for this. You want safe raw beef or raw pork? Go get it from a farm, and grind it yourself, or get it from a reputable restaurant that you can trust has taken these precautions. I have no problem eating raw meat and have done so. But I'm not so stupid as to gamble my safety on grocery store meat.

I'm in rural wisconsin and I hadn't heard about it until a few years back having Christmas with a friends family. Ive had them on 2 occasions now. Regular ground beef from a supermarket. I only ate it to not be rude, it struck me as so strange. American meat isn't unsafe to eat, it's raw meat that's unsafe to eat. This applies anywhere. Many E. coli species that are pathogenic to us do not cause disease in cattle. They can exist as part of a cow's normal microflora, and you wouldn't know unless you specifically tested for that strain of E. coli.

Yes, it's fecal contamination of surfaces of meat that's required, and yes that's gross. But keep in mind that a cow's entire hide can be contaminated with fecal flora (anything in the GI tract can be found on the skin of an animal, and cattle are particularly shitty animals). Dehiding is thus a significant source of contamination with fecal flora, including pathogenic E. coli strains.
