It's not like burning yourself with real heat, it takes a little while for your skin/the lining of your mouth to absorb the capsaicin. Especially if it's with something oily that leaves a bit of a film in your mouth and the capsaicin sticks around for a while. It's why drinking water with something spicy usually don't help and people have milk instead.

interesting. i made chef john's "nduja" once, using some italian pepperoncino in olive oil which was immediately spicy itself, but the end result took a few seconds before burning your mouth.

figured it was the other ingredients neutralizing it for a second and it really was pretty fatty, so that makes sense.
yikes, imagine fucking yourself into eating a gut burner like that because the other ingredients kept your natural warning system suppressed.
