Because it works better for American whiskey (particularly Bourbon, Rye is something else), compared to Scotch. The two are closely related, but they don't behave equally.

The other reason is history. Whiskey production in America, especially at an industrial level (particularly in Pennsylvania, Ohio and Kentucky) lines up with the beginning of refrigeration and the expansion towards the Southwest, which is significantly warmer. Thus, it was fairly common for whiskey to be served with ice, especially during the time of the bartenders of the period, who were just about the best in the world. The sling becomes the main method of consumption, and it initially called for water, because it was a morning tonic and required dilution. Once you get ice, the bittered sling becomes something that people do to cool off (it's also the root of the Old Fashioned).

Depends on the ABV. 40% stuff gets a tiny bit of cold water or nothing. 50%ish might get one small ice cube. The cask strength 60%+ gets a bigger cube or two small cubes. I try to shoot for like 45% drinking abv by the time all the ice melts.
