There are grooves which hold dirt, due to the way that they mature in the soil, it's kind of impacted in those horizontal little rings. The younger the carrot, the less the problem.
Use your good judgement. I've seen spotless young carrots that if you peeled them, shaving off a couple milometers in all directions, then there'd be no flesh left whatsoever in the middle.

Yep. You're just looking to remove bad parts and grit. Smaller varieties and younger carrots usually don't need any extra work. Larger carrots can be pretty dirty and need peeling to get the bad spots. Large carrots also tend to have a slightly bitter skin, but sweeter interiors than small ones. Nothing wrong with throwing peelings into a vegetable stock. That pungent aromatic bitterness mellows and rounds out flavors when cooked. Sweeter carrots build less interesting flavors, but are nice raw.

I peel carrots when I cook them because the peel has a different texture, but when I'm just packing a plain carrot for a snack at work I don't bother peeling or cutting it.
