Fermented food can be toxic if it's done improperly. I.e. how people got botulism from improperly fermented sausages.

The basic idea of fermentation is that you set up the conditions (temperature, acidity, salinity, etc.) so that it favors the growth of "good" microbes which will crowd out the "bad" ones. For example, classic shit like Kimchi or Sauerkraut: you add salt to the recipe to suppress the "bad" bacteria. Once the good bacteria start growing they produce lactic acid which acts as a preservative and keeps the bad bacteria out.
