The first one is solved by ensuring the fuel never reaches temperatures that can melt either the fuel or its casing in any configuration. Alternatively, if it's a liquid fuel like in thorium reactors, it melts a drain plug and spills into a cooling tray that cuts radioactivity to the point all the fuel solidifies. Even if the tray were somehow destroyed, it would just spread out and then solidify into an inert, non-water-soluble splash.
The second is handled similarly to the first: under no configuration is radioactivity capable of increasing beyond a maximum point based on geometry. In fact, a reactor breach would cause the pellets or liquid to spill out into the containment building and temperatures and radioactivity would immediately drop.
