I live in a virtual whitetopia and at my closest McDonalds they ask me to pull around to the front of the building about 40 or 50% of the time to wait for my food. The most I ever get is a Big Mac, fries and a drink (and whatever my passenger gets which is pretty much that order or less). The waits are never too long, though. Usually around two or three minutes with the longest having been around five.
