Gee, I wonder what could've caused the liver failure.
The real lesson here is to refrigerate your food correctly as a meal prepper and to not OD on aspirin, everyone should know that aspirin wrecks the liver. Do a sniff test and throw it away if it smells bad, even if that means missing a meal.
Hell, buy fucking packaged ramen as an back up if you need to.

1) AJ's fault for eating dodgy food that he smelled leading to him ODing on pepto.
2) Roommate's fault for putting food back in the fridge and not showing the medics his room so they could've saw the bottle of pepto when looking for any medications before leaving wasting precious hours they spent on testing, testing, testing.

Aspirin, motrin and tylenol ain't a joke, they are all taxing to the liver to begin with. Taking multiple times the amount of recommended dosage for that shit will kill you with liver failure with or without food poisoning.

I don't know where the pepto bismol shit from the video came from, this is the case study that the video is based off of. The guy didn't drink any medicine, he only drank water.

After school, he warmed the spaghetti in the microwave oven. Immediately after eating, he left home for his sports activities, but he returned 30 min later because of headache, abdominal pain, and nausea. At his arrival, he vomited profusely for several hours and at midnight had two episodes of watery diarrhea. He did not receive any medication and drank only water. After midnight, he fell asleep. The next morning at 11:00 AM, his parents were worried because he did not get up. When they went to his room, they found him dead.

Since it's normally soil dwelling you'll often see spores in grains, especially rice, but spores are fucking hardy so it's nearly ubiquitous. There's a concept in host-pathogen interactions called an MID, which basically is the number of bacteria you need to get to get a GI infection, it can really variate by the strain/your immune system/etc., but generally you're diagnosed with B cereus if a sample of the food you ate has more than 10^5 organisms. Refrigeration slows down metabolic processes, so they can't replicate as fast, however, they can still replicate (hence why your food and milk still goes sour over time). As for your other questions, I'm not totally sure, my guess would be a salty environment would just allow other microbes to outcompete B cereus. Toxins are typically very stable, so I doubt a weak acid like vinegar would affect the ability to make and secrete toxins. As for stews, B cereus produces toxins between a temperature of 10-43 C, as long as you're outside that range, no toxins should be produced and they will likely die of heat shock over prolonged exposure (the spores will survive though, allowing the cycle to continue).

Not sure if I need to disclaim this but I'm a PhD student, not an MD student, so take some of that advice with a grain of salt

It depends on the type of B cereus infection you have. You either contract diarrheal syndrome, which is caused by at least 3 toxins (hemolysin BL, Nhe, and cytotoxin K); or you contract emetic syndrome which is mainly caused cereulin fucking up your mitochondria as the video mentioned. Most infections are pretty mild, you either get the grumpy dumpies for a day or two with diarrheal or you vomit for a few hours with emetic. Where this guy fucked up was downing a whole bunch of pepto bismol alongside the B cereus infection, which really fucked up his liver which is normally not significantly affected by a B cereus infection.
