not in my experience. It's nearly always a fuckup on the user's part, though often times people may not notice they fucked up: my GF often lets her purse knock against the scale without even realizing it, for example.

Because of how the sensors work these things take time. First the scale has to tare itself after each item is weighed. That requires a brief pause while the machine averages out the signal from the sensor to get a clean reading. Then the same process is repeated during the actual "weighing".

You know how when you step on a bathroom scale the numbers or the pointer jumps around for a bit before it settles on a measurement? That's the problem. And it has to be dealt with twice for every item being scanned: once to zero the scale before the item goes onto it, and again once the item is actually present on the scale. If you put the food onto the bagging are too fast after scanning it you fuck up the tare. And, of course, if you accidentally bump the scale that fucks it up too. Likewise if you reach over and move things on the scale while it's trying to zero or read.

Where the hell do you live that you have to type in the sku for steak instead of just scanning the barcode? On a related note about barcodes, when you are buying a price-per-pound item such as steak, look at the upc number below the barcode, somewhere in the string will be 3 or 4 numbers that match the weight of the item as listed on the label. Use the PLU entry screen like you use for bananas, type in the upc from the item, but replace the 3 or 4 numbers with a smaller weight like 0323 instead of 0623.

Worked produce for 3 years. We ballparked waste and only kept inventory on higher cost produce like passionfruit, those shitty salad bags and misc seasonal 'exotics' like mango.
