Mine is probably a bit on the mundane side.

>Be me, work at small fast food restaurant
>Notice the Dr. Pepper is low, so I decide to change it
>For those unaware, fountain drinks tend to come in boxes like pic, with a bag of the syrup inside
>Sometimes, it's easy to rip the tab open, sometimes they're bitchy; Dr. Pepper tends to be particularly irritating
>Tab doesn't rip properly, think about using a knife
>There's labeling on the box that specifically says to not use sharp objects
>"Pfft, I know what I'm doing," I think to myself as a lightly stab into the box
>It starts leaking
>Oh god, it's leaking
>Who could have possibly seen this coming?
>Frantically run to the sink, alert assistant manager to the leak
>Don't tell him it's me, naturally, just say it started acting fucky when I tried to attach the hose
>He looks at it for a bit
>"Oh, anon, I figured it out."
>"Y-yeah, manager? W-what do you think it was?"
>"See here?" He points to an area near the plastic bit that connects to the hose. "The seam is broken. Guess it must've been weak."
>"O-oh, ha ha, you're so right, manager! That makes sense."
>He manages to seal it with duct tape, sort of. The Dr. Pepper comes out extra foamy, but is otherwise fine.

It wouldn't have been a firing-level offense, especially since I was a pretty reliable worker (which is rare in fast food), but I'm sure I would have been reprimanded for being a loose cannon retard.
