Apparently they're a particular kind of noodles that are only produced in the Fall River area. But yeah, it's exactly what it looks like: a big plate of food with a little hat. (And the bottom half of the bun underneath.)

You can order it all wrapped up so that the noodles and sauce have time to congeal together into something that's slightly easier to eat as a sandwich. I'd never heard of this before I moved to CT and started reading up on regional dishes. Drove all the way to Fall River to have it. Wasn't impressed, but glad I tried it all the same.
