generally speaking, pipe smokers don't inhale, so the smoke doesn't really get into your lungs the way cigarette smoke does. the few that do inhale also tend to be cigarette smokers, and even they inhale only rarely (one or two puffs during the course of the entire bowl).

smokers of spanish style cigars also rarely inhale (unless they enjoy throwing up), but smokers of lower end sweetened cigars such as white owl and swisher sweets inhale at a much higher rate.
