There's a Lolita general thread which is great for questions like these.

Their 2L varies from like 92cm bust to like 106 cm bust. 4L is like 98 to whatever they damn well please. They have no standard sizing at all and it's determined by voodoo. Look at some of their full-shirred items, I've seen them worn on bigger busts. But honestly, Bodyline isn't too great about working on larger sizes. To be truthful, you might have better luck with fully-shirred brand (Check out Chantilly and MAM). I've seen some pieces go up that high.

You'll have a lot more luck finding existing things that fit you (although there are options, but they stretch horizontally and are cut for small cup sizes regardless) once you get down to below the 110 cm mark. I started there and I'm about 100cm right now. I still have issues with some Bodyline pieces but I wear brand just fine.

Not always. I've always seen it as a catch-all term for talking about the entire community, or when someone just doesn't want to get super-personal their sexuality. E.g.
