I've seen one in Adventure Time cosplay threads. It is well done, iirc the wig is a bit underwhelming (but then again it isn't possible to do her hair like the show so that's kind of a moot point).

Foam batting and a lot of it. Shape it the way you want, sew it into the wig and cover it with wig fiber. Hairspray the fiber in place.

I also advise spray painting/dyeing the batting to match your wig as close as possible so if any of the wig fiber shifts there's not a giant splotch of WHITE just showing through.

You can either line the skirt completely, but that will add weight and is considerably more expensive.

Or, you can use a baby hem:
