The dress is okay, but it's one of those that can easily look super ita given the right coordinate. Here's what I would fix about this outfit starting from head to toe:
-She's clearly trying to go for Gothic lolita. More appropriate headgear like a circular headdress (with a veil) or a classy hat would really improve her look. Bigger hair and a bigger headpiece will also balance out her silhouette.
-Her face is very round and her jawline is undefined. The way she's done her hair only exaggerates that fact. She should get her hair cut with lots of layers for added volume. The layers should frame her face for structure and softness; it would also define her chin and jaw. Her hair needs styling to. In this picture it's just hanging there and looks sort of limp and unbrushed. Even just curling the ends of the hair straight under makes a huge difference in volume and shape.
-She should try some natural makeup. For her eyes: a color close to her skin washed over the whole lid, crease defined with a neutral brown, browbone highlighted for lift. Mascara. Blush on her cheeks. Tinted gloss or balm. Her skin seems pretty even from here, so all she would need is a tinted moisturizer and perhaps some concealer here and there. This is easy to do, looks very natural and will make a huge difference.

I'm a lot closer to Tiffy's age than most lolitas, and like her I don't have kids. Yet I still managed to get all my snowflakey-ness rubbed off. (And holy fuck, was I a snowflake when I was younger. I'm amazed anyone tolerated me.)

Having kids can help break through that, but not always. In my case, it was hard times in my 20s that forced me to wake up and become more attuned to other people and the environment around me, rather than living in a fantasy world in which I was the only person who really mattered. But some people just never get it, no matter how many hard knocks life gives them, and Tiffy's one of them. She's like a snotty 15-year-old in a 48-year-old body.
