IIRC she has PCOS but seems to be using it as an excuse for why she is the way she is, and that there's nothing she can do about it. PCOS makes it easier to build muscle though, so if she does have it the fact that she mainly has put on bodyfat suggests she's not putting in the work and not eating in a way that is helpful for building muscle. Too much junk, not enough protein. The bruises are completely normal for the kind of training she does though. But you're right she is often quite pale. This is just something that's different between a cotton blouse and a blouse made out of sheer material. If you put someone in it who exceeds the maximum measurements of the blouse it likely won't rip if it's cotton, but it will likely rip if it's organza/chiffon/etc no matter what kind of seam you put in. The fabric is just too delicate/loosely woven.There are types of sturdier organza used for skirts/dresses but they're not used for blouses because they are way too stiff. I have no problem with these blouses being sold but they need french seams, a disclaimer stating it's a delicate fabric and a warning on the listing stating that you must be sure you're (insert amount here) under the max measurements.

I've seen quite a lot of street snaps (as well as brand adds) of well dressed lolitas who hold/pull out their skirt, although often only on one side (when the other hand is holding a bag) and the skirt isn't pulled up/out to the extent these people are doing. sensory issues can also include being sensitive towards pressure of elastic waist bands and tights on the waist. This isn't exactly limited to people with autism though, anyone can have this.
I personally really hate the pressure of outfits that have the pressure of the elastic waistband of a petticoat, bloomers and a regular or elastic waistband skirt on top. There are ways to fix what's causing the issue though without giving up the poof.
-wear skirts/dresses of stiff fabric that have an in built petticoat (layers of tulle stitched onto the skirt lining), this will usually make an additional petticoat unnecessary unless you want mega poof.
-remove the elastic waistband from petticoat and bloomers and put on a yoke or curved waistband instead that sits a bit lower than your anatomical waist. Size flexibility can be achieved by adding a drawstring on a portion of the front waistband.
With the waistband of the dress/skirt sitting on your anatomical waist (or higher) and your petti+bloomers sitting lower you can avoid stacking pressure on top of pressure. The removal of the elastic also lessens pressure.
-if you hate tights like I do, just don't wear any and opt for socks instead.

I used to be a little picky about waistbands of underwear and pyjama bottoms and stuff like that but still wore jeans made out of very thick and stiff material without it being too much of a problem. Then I got IBS and 3 separate bouts of stomach ulcer and that was it for me. Definitely had an effect on what I preferred to wear. When I wear jeans now they're either too big in the waistband, stretchy, or have soft and stretchy elasticated waist bands. I got even pickier about underwear and PJs.
I tried on an outfit that had bloomers, petticoat and skirt with elasticated waistbands and all of the pressure of the waistbands were stacked on top of each other and I found out I could only wear it for a pretty short time until I felt I needed to escape from the clothes.
Then I found a video by some historical fashion youtuber who said she patterned her petticoat and bloomers to fall below her waist a bit (and of course no elastic) to avoid excess bulk at the waist, and used a yoke or curved waistband plus drawstring in front (and open fly) and this was a bit of a light bulb moment for me. It's a fairly simple alteration if you have or can make a yoke pattern.
