OK, I looked up the character this time and found the actual model. You can wear a normal bra with this, or a low-back bra if want the bow to hit the same place on your body. Just have the bow cover the bra band. I would find a way to secure the bow so it doesn't slip off and expose the band, e.g. a loop on the shirt to thread the band through or tie the bow to the band itself. I have a few shirts that are cut in the back like this and they cover my bra just fine.

Anyway, I think you are underestimating how much the cape will cover your back. Unless you plan to be standing somewhere windy all day, your cape is not going to fly up so much that your upper back will be exposed.

If you are so autistic that a little bit of showing bra is going to completely ruin your cosplay, then sure, do the extra work and sew padding into the shirt. You would wash it in the same manner that you wash all your bras. Alternatively, the padding could be inserted into pockets so you can remove them if need-be (like how you can remove the padding from some padded bras).
