The best way to do that is to pick an interesting blouse - Pic related is a few different ideas for blouses that can work in an OTT coord, they either have a lot of detail or several different colors that can match your coord; I went with AP to make thing simpler for me but you can go with any brand that suits your needs.
Another thing you can do is wear large accessories with a blouse that you already have to add interest and detail, like a wide ribbon around your neck, a frilly jabot, an eye catching necklace, etc., and you can also use a bustier (like the Horror Garden one) to play around with layers.
