My suggestion for the holes would be to buy a set of curtain eyelets, like in pic related. There's a nice vid here https://www.youtube.com/watch?v=CV3a92DikEo of how to attach them.
As for the sleeves, I'd personally interpret those as being quite a chunky ribbed knit fabric. If you'd rather them by 'flat' like in the pic, you could try to find a white fabric with thin black stripes (ticking fabric?) and dye it purple, since obviously the black won't take the dye but the white will.
