The helm is nearly 100% accurate. pic related, showing the nasal attached to the cammaille and unlatched, from a German effigy of c.1350 Freiburg Cathedral.

Why? For the vast swaths of men on the field, undyed was just more cost effective. Furthermore, exposed gambeson is not uncommon, and depending on the thickness, may be a quilted defense in its own right.
