I think there was only ever that one instance of an idol shaving her head, but if I recall correctly, in groups like AKB48 it's against an idol's contact to be in a relationship publicly and you can be kicked out of the group. (or has that rule been changed?)
And the rule doesn't apply to all Japanese female entertainers, only specifically to idols because they're supposed to be marketable to borderline-pedophilic old men by being cute and pure and not having a boyfriend. Last year, an idol (pic related is her) was stabbed 20 times by a fan just because she wouldn't accept his gift.

Most girls are really young when they're forced into the idol industry by their parents, but a lot of girls also become idols hoping it'll be their stepping stone to fame.
Doesn't pay well at all, especially not for lesser known groups.
