There's a pattern floating around somewhere on google someone made for the head. I literally just had to print the pattern out on a large enough sheet, trace the piece on to card paper and fold it, then draw on it/cut the eyes out and tape some seethrough cloth on the inside.
Pic related its me, sorry for the bad picture this was at like Pax 2012 and I didn't have a proper camera. Tim Schafer though!
