The fatal flaw is you not pushing the product to market as quickly as possible and fixing bugs in real time. If this is a web project, you are doing it wrong. If its actual software or a game, then I can see taking this long.

Nothing right now, I thought I was close to finished but it looks like a huge chunk of code I've written is incompatible with some 3rd party APIs I was planning on using.
