There's a reason some graduate schools only ask for last 2 years GPA, final year GPA etc. Your overall performance over the last 4 years is less important than your recent performance trend. A shitty final year with aced 1st through 3rd is many magnitudes worse than bad early grades but solid late game grades.

A friend of mine with a near perfect GPA got rejected from pharmacy school for having one bad grade in his last semester. The rest was virtually all A+s. He asked the admissions office or whoever makes the final decision and they told him he seemed to be on a downtrend.

Not sure what manual labor you're talking about? As for the low acuity stuff (cutting abscesses, sewing stupid lacerations), that gets done by the 2 or 3 PAs working under me at any given time. I'm usually only seeing ESI category 1 and 2's, for whom the medical decision making that i do either shepherds them through the first (and most critical hours) of their illness or gets them killed.
