What happens if exchanges run out of BTC?

It would be extremely painful.

You would not be able to purchase any BTC on that exchange.

It's actually quite scary how little BTC is on exchanges right now. 70% is being hodl'd in cold storage for over a year, only like 2 million coins are on exchanges and many of them belong to people that don't intend to sell. You could wake up one day and just see a giant green candle as sellers dry up and relatively small buy pressure eats up all the available supply

Nothing. Exchanges exchange crypto, they don’t hold it. The only reason they hold any is people left some there for convenience.

They start offering the equivalent of an “IOU”.

That’s how markets work. The price goes up and attracts more sellers. The real question is what happens when no one wants to sell for fiat no matter what the price because it’s a bad trade
