They virtually just buy or sell BTC over the counter (not in the open market) to balance the funds. You can expect them to be making quite a bit of money from the OTC trades since the ETF does not track the price 24/7 like actual Bitcoin which means that they have quite a bit of leeway both in which investment firms they buy the BTC from but as well as when they do it.
