I misworded. Basically if you dont have ins in America you are as good as dead. Its also super expensive so most people have to get it through their work. But if you don't like your plan, you only have like 4 weeks a year to change it per IRS. Just the good ol IRS looking out for the taxpayer as always

employer offers health, dental, vision, life, etc. insurances. you choose which ones you want. usually there are tiers e.g. there's a cheap health insurance plan that covers catastrophic shit with an insane $10k deductible or whatever, there's a mid-range plan that's a bit more expensive and has a better deductible, and there's a plan that covers a lot more but costs a fortune. you choose which packages and tiers you want and then you're stuck with them for a full year. by law you aren't allowed (or given to the option to) change your enrollments unless you have a "qualifying life event" (marriage, baby, changing jobs, etc.)
