People living in small towns will generally have a family and friends support system, which keeps their mental illness from escalating. If it does escalate, family and friends are more likely to pursue treatment for a family member.
People generally move from small towns to big cities, so do the mentally ill. But when they arrive the support systems they had are gone.
The mentally ill who decide to self medicate will almost inevitably move to the big city as it's easier to buy drugs, and more socially acceptable to do them without family.
The deeper a person gets into their respective drug subculture, they more they think their degeneracy is normal and reasonable, and so are more likely to do it it public. That drug culture doesn't exist in small cities and towns. Outside of the occasional methhead.

Housing is definitely an important step in rehabilitation, but it's one of the last.

Drugs come first before the loss of housing, and drugs habits have to be kicked before housing will do them any good.

Spent a month in Toronto homeless when I was 17.

It's pretty obvious at this point you've haven't spent much time around the mentally ill and heavily addicted. Soend a couple hours around a methhead or down junkie and you will quickly realize the mental processes required to live in a house, unsupervised, don't exist. Giving out free houses to these people will only result in a lot of fucked up houses.
