Do I need permission from the copyright holder to sell items containing their copyrighted symbol on it?

I'm guessing "yes", but what if the symbol is a fictional one? The one I'm thinking about is Game of Thrones symbols.

Yes.

OK, so how much does it cost to get the right to sell then? Or what % of profit do they want? Usually/average that is.

Between 0 and 100%

Why don't you make your own symbol?

The price would be entirely up to the copyright holder, or at least whatever price you could negotiate with him.

how about you take their symbol and make a slight adjustment to it.

Fuck it. You could probably make $10k in revs before the cease and desist came knocking.
