So a friend of mine has millions of dollars worth of BTC, but can't get any of it in cash because he hhas to "Pay lawyers tens of thousands of dollars" to sign off on some transfer.

This sounds odd to me.

Is it actually this painful to transfer large quantities of BTC into US Dollars or has some bamboozling occured?

Sounds like a bunch of bullshit unless the only reason he got the BTC in the first place was through some agreement with someone else, so he doesn't have any BTC, that other person has it, and he needs lawyers to sue that person to get ownership, or some bullshit like that. Even then, if he had a solid case lawyers would do it upfront with no initial cost because they'd be paid after, or he could just get an unsecured loan.
