I see two potential scenarios for a punch. either:
A. the vein compresses and ruptures on impact
B. the low mass padding detracts from the blow offer some cushion with no additional mass.


in fact the pic related is a strong hand. But, theres fat padding you say? Simple physics mate, the fat, does at padding, but unlike veins it adds mass.

In fact, consider boxing gloves, boxing gloves are more likely to cause concussions than a bar fist hit. The added mass of the padding causes more snap and push on impact. All the padding does is prevent bruising and cuts as the impact is spread over a greater area -- though the total magnitude is higher.

So while this boney skeletor might knick your brow with a proper strike, a real meaty man paw would leave you unmarred externally, but the concussive impact will lay you out potentially with a single strike dealing a greater deal of cranial inertia.
