If I hold a patent for a drug combination in iran, can the USA pharmaceutical companies usurp me and copy me and not pay me the dutifully owed monies? Since Iran Is apparently enemy of USA?

Not a lawyer but I don't think so. It doesn't mean that they can't produce the drug, but they would need to prove they invented it before you to make that claim.
