I can't imagine how much that must hurt. Like his skin is just disintegrating - look at his back. Dang, I feel sorry for the guy it can't be a happy existence.
How do his legs bend straight out like that, anyway? Does the fat displace his joints or make him more limber? Or is it just the weight pushing down over days and months that just stretches out the ligaments and muscles?
Also
Does this show ever touch on how much and how often 600 pound people must shit? I mean someone must be feeding him day and night for him to maintain that mass, and that much food in means he must have monster shits several times a day. It can't be an afterthought, he must be shitting all the damn time. Clearly he isn't getting up to use the toilet, how does this work, logistically? And since he's also not going to the store or preparing his own meals, does everybody like this have someone else bringing them food and cleaning up their shit all day long?

Anyway, yeah, I feel sorry for the guy. Of course these are the consequences of his own choices, but mental illness is sad.

They shit themselves and their caretaker wipes it up.

it's a messed up world when something like this is even possible.

They are. These kinds of people, if immobile (or heavily immobile) have enablers. Though today there's always pizza delivery, so they don't always have an enabler, though they often do.
