Usually very niche or bespoke porn is what fokls pay for. what indie porn actors do nowadays is they'll set up a day for filming and take private requests (with a very high price tag) for like really niche fetish shit, it might not even be that edgy, just idiosyncratic.
I don't know how the bigger studios work, I assume they're either money laundering operations or the strategy is to produce the content once, spending a few grand in one day and then pamp eet across various distribution networks (like MindGeek) - paying to get exposure, then after a while reediting it and changing titles.
