There are two types of narcisism, and NPCs can't really tell them apart. One is a mental disorder, the other comes from incessant winning in life (Trump for example) and is actually warranted (you just statistically speaking know better and your time is more valuable).

To expand on this, I'm pretty sure it comes from the natural human capacity to assume averages at scale and it comes from the same problem as "my vote doesn't matter"
When you don't have the brain power to actually assess every individual person, your brain boils it down to looking at an ant colony. Realistically though each person has their own goals, is going somewhere, and taking advantage of tools society has given them to perform tasks that hopefully give them some sense of fulfillment. For example you feel your own body and feel like you're "one" thing, but really you're millions of things all working together for a single purpose and you don't perceive the actions of each cell but every cell has its own agenda and each one matters equally. Your heart never calls your liver an NPC just because it doesn't perceive it's perspective.

That's a you problem, not a society problem. Nowhere in the world is it optimal to be good at everything, in fact it's a detriment. the real world rewards specialization and as a result efficiency.
