The government does pay off its old debt, but continues to take on new debt at such a rate that the total debt balance keeps increasing. Obviously though, the more new money gets created the worse inflation gets.

My understanding was that the "printing" happens when the federal reserve buys the debt since they don't care if it actually gets paid back.
