To earn the really outrageous salaries in America you need to live in areas where the cost of living is comically absurd and also deal with companies offering shit healthcare plans. The number comparisons you are seeing do not display the reality of such pay differences.

Also, small note on the bit about where you need to live to work here - ALL of the high paying cities are nightmarish to be in. I have been around burgerland, there are actually some nice places to live there. But most are not easy for large salaries. The ones that are good tend to be under constant invasion by the rats that ruined California/Chicago/etc.

Yeah, health care for most Americans is probably equal or better than western Euros. If you are in the top 50% or so income wise in the US, the extra amount amount you spend on health care is still probably about the same or less than the extra amount Euros would be paying in income taxes, and the quality will be about same or easily better. Americans in the bottom 10% or so get all their health care paid for by the government anyways, and will be on par with western euros. Now, that lower middle 40% ish might be worse off than Europeans.
