options are priced to reflect a risk-neutral position. the MM will calculate what price they think will negate their hedging losses, and put a bid / ask around that theoretical value. so if they get filled at the offer or bid, they expect to lose on their hedges (or, vice versa, win on their hedges / lose on the option) everything except that little amount between the bid/ask and theoretical value midpoint. in theory, anyway.
if they are wrong, and the underlying moves more than they expect, they will lose past a point. if they are wrong and the underlying moves less than they expect, they will win. overall though, they will tend to make the spread, as over huge transaction quantities and lots of time, their trading wins/losses will even out.
