You are not taxed on appreciated value until your gains are realized. Meaning, if you buy XCoin at .15 and it moons to 15m you are not taxed on that appreciated value until such time as you solidify the gains. This could be cashing out, ie you sell your XCoins and are now responsible for the profit of (15m*x)-(.15*x). This could also be TRADING INTO A DIFFERENT COIN. So if you bought 100 XCoins at .15 but later traded those 100 XCoins at 15m for the market value of YCoins you have realized your gains. It doesn't matter that the money didn't go into your bank account. You have realized capital gains on your investment. That is a taxable event.

Exchanges keep detailed records and either automatically give or willingly provide those records to governments. So if there is any PII that ties you to those trades -- you signed up for a site, the money is sent to a bank account with your name -- then you can, and likely will, be held responsible for those gains.
