I got a near 70% raise on an already good salary with just 12 months of experience fresh out of school. Took several rounds of negotiations but worked out just fine.

Just present the manager/CEO with a list of your significant tasks/developments/achievements/responsibilities. Bring offers from other companies, don't accept the first offer. Always consider the secondary benefits.

This is OP's (former) supervisor. I would like to re-iterate this point for the benefit of any other would-be revolutionaries. What has worked best for me is to use the word “flabbergasted” a lot during the conversation. It has quite a few things going for it:
- 4 syllables makes it a big word
- using big words = smart person
- it is a very classy word
- people don’t hear it often so doesn’t carry the buzzword context
- it is fun to say
- very versatile word

So it would like this for me:
>thank you Jim for taking time to see me today. Really, I’m flabbergasted.
>I know your time is valuable so I’ll flabbergast you by cutting right to the point
>Jim, like you, I have a bunch of kids
>I need money Jim
>Yesterday I was flabbergasted to find a $0 balance in my checking account
>the only way this could happen is you don’t pay me enough Jim.
>Jim, let’s both agree to be flabbergasted here and go with say 25% increase
>flabbergasting to meet with you today Jim, totally flabbergasted here as I’m sure you can tell as smart as you are.

Also of note I use the other party’s name a lot. People love it when you connect with them that way it really helps.
