The basic stuff has to get tested and proven to hell and back for penalties and reputation to make sense for node operators and contracts owners alike. All the low hanging undesired unmalicious failure scenarios have to be found and fixed. I assume the hundreds of teams have been finding and fixing/reporting these things as they go, but most of us have no way to know how well that process has gone.
