I'm fairly new to /biz/, but I have some questions regarding credit after being denied for a new line of credit recently.

1. The reason I was denied was because of "past or present delinquency," I looked on my Experian credit report and saw that there was a charge that I didn't make. As it turns out, it was linked with my name but not my SSN, meaning it was likely my father's debt since we share the same first and last name. How do I prevent shit like this from happening in the future? I don't want to be applying for a mortgage or something big in the future only to be screwed over by my parent's terrible finances.
2. I'm trying to get a credit score of over 800, right now I'm at about 745 or so. I know the standard definition of credit utilization. However, is this based on how much is used throughout the month or the balance that shows up on the statement? I've always paid off my balances as soon as they appear on my online portal before the actual due date. How does this affect my credit utilization ratio?
