Zack was with Aeris, not Tifa. i don't think Tifa knew 100% that Cloud never made it to SOLDIER - she just knew that something wasn't adding up, but she didn't want to say anything because after all, Cloud was the strongest fighter in their group and was leading the whole fight against sephiroth and Shinra. Cloud had promised Tifa as children that if he ever made it to SOLDIER and she was in trouble, he would come rescue her, and Tifa wanted that to be true, as she was in trouble

Cloud wasn't -all- wrong about his memories. He was in fact there when Sephiroth went to Nibelheim the first time - he just self-inserted himself as Zack (who was also there in SOLDIER), but in reality, Cloud was just one of the regular Shinra grunts in the blue uniform

Cloud did in fact kill Sephiroth after he attacked Tifa and stole Jenova from the reactor. Cloud got stabbed by Sephiroth, but managed to throw him into the Mako reactor and kill him

you have to remember Cloud got fucked up when Hojo was experimenting on him exposing him to Mako and Jenova cells - so his memory was kinda fucked up. Zack was the one who saved Cloud from Hojo, but Cloud had mako poisoning and was all fucked up. So as Zack died saving Cloud - Cloud basically assumed Zack's persona, partially because he was fucked up from mako poisoning and part because Zack was in SOLDIER and basically who Cloud wanted to be anyway
