Kinda. The underground civilization in the first book is a multi-tiered city developed over hundreds of years after an apocalypse occurs that is technologically advanced in it's own right, UV panels on the ceiling that aid circadian rhythms of the citizens, vertical farming that uses the excavated minerals to aid plant health and growth, enclosed water and air purification systems, hydrogen powered, advanced medical diagnostics, ect.

Due to environmental and social selection factors from succesful (and lucky) miners spreading their seed, the population becomes pale, short and toned in muscle density, which also means the females have birthing hips. Not to the extent of dwarfs in Tolkien's writings, around 4'6" - 5'6".

However on the line of thought of Demolition Man, it is common belief that the surface is unliveable. In reality, the fiscal elite live above the surface while the mining population under neath toils for neoplasmin and stone for their benefit. It's revealed relatively early because one of the protagonist makes her income by smuggling the underground wealthy to the surface.

I haven't really discussed vaccines in the book. Maybe there are none. ¯\_(ツ)_/¯
